Workers Compensation

If you are a worker’s compensation patient, please know that we are very adept at obtaining authorization and providing the necessary documentation to get your services authorized.

Due to special authorization requirements, the worker’s compensation process can require a longer period of time to certify. Rest assured, we are very knowledgeable in this process and we will do our best to get you scheduled as soon as possible.

Disability Form Completion

Filling out all of the paperwork in a timely fashion will help us with your claim.  Please obtain needed forms from your employer or insurance company.  Complete and sign your portion of the form and turn into our office.  We will then complete our portion and send the form out to the respective company.  There is a $10.00 flat fee for disability form completion per physician per injury.  Should you need assistance, don’t hesitate to call us.
